I recently had a problem with my in-game voice chat making me sound like a Cylon off of Battlestar Galactica. What was happening was my mic would play back and loop everything that would be transmitted. This would produce a screeching sound if I were to hold the voice-transmit button down long enough.
To fix this I looked at my Realtek HD sound options. In it there was a 'stereo mix' option which I muted. Along with a muted mic to prevent that echoing sound whenever you talk it now works great.
