As far as I know tf_medieval_thirdperson has been client only and barely provides any advantage besides maybe a quick corner peak on B, but that's not much of an advantage anyways. Besides, medieval thirdperson doesn't require cheats or external plugins.
As for the other servers, I see thirdperson is a bit of a problem as it allows for campy playstyles, not to point anyone out but a certain player used to play pyro and wait for players to come by only to ambush them and burn them when they least expected it.

-1 Because it makes for very unfair playstyles for some classes, and utterly counters spy, and some sniper peeking. Sure, you can do it with taunts, but taunting cripples you somewhat.

I've never liked 3rd person. But i'm not on the servers very often, so my opinion range is rather small.
