Instructions:

Extract the zip file and place the two files (skullz.vmt, skullz.vtf) in the fallowing path:

Steam\steamapps\<username>\team fortress 2\tf\materials\VGUI\logos

When you've put the spray in the right place, you still have to select it through your options menu...

Maximum spray size is 256x256, sorry.
