Alright I have a similar thread in the TF2 section showing my BlackMesa game failing, and this is a lot like it.

Here is a link to the other TF2 thread.

Check out these two albums and tell me what you all think of the fail game.

*UPDATE* Just so you all know, this is a rare event for me. Only has happened once in Tf2 and that was Blackmesa and twice in DoD and those are the pictures you all saw. So don't think this is what I always see. Just a very rare event.

Cause its true, only thing you can try is removing the heatsink/heat shroud off the graphics card and cleaning it out w/ canned air. Other than that you'll need a new card.

It is your video card. Also, under your forum name it shows you play TF2 as well. I hope you realize that DOD:S and TF2 are both on the same engine.

overheated video card = fails lol. and prolly really old... try updating the card. otherwise u need new 1 :(

I had this happen on my old computer. We were having weird glitches like this then the computer would freeze. We took the case off and looked at the card since tech support said it was a hardware problem. The plastic fans on my nvidia 5950 ultra had MELTED ONTO THE CARD. That sir, is fail.

Old card Need New one asap. If you cant afford one at the moment try Lowering your settings at lowest possible and Make sure you Do NOT enable any AA = Anti Aliasing
See if that will help you gaming until you buy a new one.
Try playing at 1024x768 with everything at low but from what i see you may want to go all the way to 800x600. Hope this will help you.

Peace.
