Ok, so the round finished and then we swap teams as normal well, I swapped from red to blue but I was blue in a red base? glitch maybe.

I use that hack all the time. Its very effective.

Seems to me you have a gfx glitch. I would try either running TF2 in a window by using the "-windowed -noborder" command line switches; recording a demo for a few seconds to reset all the video stuff; or knock it off with all the alt+tabbing. I alt+tab all the time too and found running TF2 in a window prevented 99% of my problems. Of course it's fairly system intensive to do this so it may not be the solution for you.
