-Just use this simple request form :

Theme : (eg Grunge, Team Fortress 2)
Colours : (eg Dark colours, green, reddish colours)
Pictures : (any pictures you want using/implemented)
Text : (Any text/info you want on such as your name)
Extra : (Anything extra you want)
