SprayR (http://sprayr.scumhunter.net) is the easiest way to make them.
The sizing limitations of sprays are: 16, 32, 64, 128, 256. For bandwidth concerns, the largest square image size is: 256x256.
If they are animated the 256 is limited to 5 frames only. You can make transparent GIFs and the transparency transfers to the spray, like my PDGAY spray.

You can pretty much disregard the TGA and animated texture converter method. It is much easier to create GIFs, animated GIFs, JPG, BMP and import them into SprayR, which does all the work for you.
Sprayr will put the sprays into this folder for you:
Valve\Steam\SteamApps\\counter-strike source\cstrike\materials\VGUI\logos\
After it creates the spray, launch CS, goto your options menu and your spray will be in the drop down list. Choose your spray and re-connect to the server (if you were connected) to make it work.
Your final spray needs to be less than 130k. SprayR will tell you if it is too big. If it is too big, then you need to remove it from your Valve\Steam\SteamApps\user_account_here\counter-strike source\cstrike\materials\VGUI\logos\ direcory, and edit your image or animation so that it will be the correct size, then try again.
This is the easiest way I have found. You can convert video to GIF, then edit the resulting GIF down to the proper dimensions and file size, or just grab an image off the Internet and convert it, what ever you want to do. I did a screen shot of one of the Volkswagon "German Engineering" commercials, resized it, added text "Ow Snap!" "Pwned!", and then created a spray from it.
