During the period of the Black Death and the Great Plague of London, plague doctors (physicians) visited victims of the plague.

I believe the great bucaneering era was around the 17 to 18th century, so a plague doctor (especially an english plague doctor (England being the major supporter of bucaneering agaisnt spanish fleets)) fits in the context.

True, but perhaps the most famous and most successful single pirate was Francis Drake who operated in 16th C. Also, THE plague was 14th C so still pretty far apart. I suppose they may have continued using the get-up with lepers, though,
