How does this happen?

See double entry for me killing Tibby. How the heck does that happen?

simple: you both did damnage to each other, then you hit him with that demoman melee and you took explosive damage + fall damage, then died after you killed him. Hence him "finishing you off."
Hopefully I explained this well enough.

I believe he's referring to the self-assisting bug. Basically valve decided that you and your spirit are two different people, so when you are alive you do damage, and when you're dead you're a separate entity doing damage. They claimed to have fixed that in patch notes though.

Um, looks like Genshin assisted his own kill. He wasn't finished off by Tibby.

read the tf2 changelog from a few updates ago they "fixed" the assisting self bug.

One way is to throw jarate on them, do a little damage to them, and have them finished themselves off. I don't know if there are other ways.

You can do it yourself by throwing jarate on them, hitting them with the Tribalman's Shiv, and letting them die from bleeding
