Request Twitch stream overlay

So I'm getting into streaming more(notice my streaming activity recently) and I want to make an overlay for it. Can it not be my eGO name and the twitch name(mason5040), with the logos of the games I play? The games are battlefield 3, gmod( can it be the GMOD G with GMOD below it?), CS:GO, Planeside 2. Thanks if you can! (You can put a made by: in the bar if you wish, you deserve it.)
