I just brought up paint.net, cropped a Minecraft screenshot, and slapped on a few text boxes here and there. (Amazing, I know.) I think the Minecraft shaders did most the the work for me (the Depth of Field blurring the background and focusing on the character, also the lighting and water look awesome).
