guns disappearing

Please describe the issue in as much detail as possible.
basically guns randomnly disappear at times, i've killed ct's and the guns just randomnly disappears, it doesnt happen with only that, it can disappear even if u just throw the gun, it's common to disappear in guntoss.
What should be happening instead?
it shouldnt disappear
