I finally got it

I became a Dedicated Supporter when I officially joined the community. I did so out of absolute appreciation for the eGO server I play and the diverse community of people that I find phenomenal. In eGO I found fun, laughter and great times in the entertainment. I gained meaningful connections, some of which have turned into real offline friendships, but also a venue that provided a distraction from stressful moments of life that are inevitable. I have long wanted to display the DS medal to show my deep appreciation to the community. I want all to see that this place is "THIS" special to me, and today I can. I am a Dedicated Supporter and I want this place to continue. I am grateful.

Most awards are given at leadership discretion as they see fit. (a handful are automated).

You have to ask for some. Time played awards and the "Titan Of <game>" are by request. Most are by leadership discretion as Simon said and as far as I know, only Sr. Managers and above can actually award them.
Time as a member and Dedicated supporter are awarded by the system
