Sometimes cases of friendly fire happens, usually when someone is auto switched. Im not sure exactly how this causes friendly fire. But it has happened to me before.

The thing is that neither of us got switched to the other team.
His sentry kicked my butt when I wanted to kill a RED Demoman, I jumped infront of the sentry and got its rockets in my back. Let me tell ya, that hurts. Alot.

Other teammate's rockets and 'nades don't hurt me either, that's why I was so confused about this. Why should the engi be able to kill his teammates? This is stupid...

I love how the engy never returned, nor how nothing else happened on the whole server, even speech.

the one time i saw a case of friendly fire was on me, i walked infront of a sentry and it started firing at me and the engi on my team just stood there D:

The sentry rockets can kill teammates, it does such a insignificant amout that the person has to have < 5 hp to be killed; the senrty rockets do not always hurt either. I do believe it is a glitch.

Yeah, I've never actually noticed this happening, but I was on my hlstatsx page a couple days ago and noticed I had 1 teamkill. Didn't think it possible, but this would explain how it happened.
