Signature request

I was hoping one of the great artists down here at the art section could whip me up a sweet sig for me with this picture:

Details

• Obviously much smaller, like signature size.
• And id like it to say:
  1. =(e)=? Chase
  2. CS:S Division

(Id like the text to be on the wall, to the right)

Also, rounded corners would be nice, if its not to much i mean
