It's generally used on Avalanche to refer to suicide-bombing somebody with a lit frag grenade, popularized by a player named CRAZY HORSE. Usually, there's also a quick flashlight burst as a warning, but you can always be sure of who it is when somebody drops down from a building and bull-rushes you.
