Now I need to assist kill myself while dominating myself. Shouldn't be too hard...

You can reproduce this pretty much every time. Jarate someone you're about to dominate, then have them kill themselves. You'll get a "double" domination every time. Now the real question is, do you have to get revenge twice to clear the double?

Oh yeah, I'm pretty sure you have to physically damage the person too.
