Server crashes after map ends

Please describe the issue in as much detail as possible.

After the map ends, the map loading menu will go infinitely, until you disconnect back to the main menu. Try to load the server in the community server tab, and It disappears until the next map is loaded.

What should be happening instead?

Map ends -> Load into next map -> No one cries
​

Please describe how to replicate the bug in as much detail as possible

Join server and wait until map ends

I'm not sure what is causing this issue, but it seems well known among many players.
