I'm not sure what's up with the medic helmet. If you don't know your history, it is the common German infantry helmet used in World War I. Why would valve use a hat from World War I in a game like TF2? Interesting.

As a mostly standard American with little to no understanding of German culture I can say that the WWI infantry helmet is by far the most distinctive of any German hat to outside viewers. It has a remarkably similar relationship with Germany that the Ushanka has with Russia.
